An application for a permanent resident card for the family member of the EU citizen

The family member of the EU citizen (non-EU national) shall apply for a permanent resident card for the family member of the EU citizen before the expiry of the residence card of the family member of the EU citizen (on the residence card see the section The application for a residence card for a family member of the EU citizen.

The application shall be submitted in person on the official form in the province office. The application should be completed in Polish.

The issuance of a permanent resident card for the family member of the EU citizen is subject to a fee of 30 zlotys. The fee is payable on the bank account of the of the relevant province office where the application is submitted. The bank account numbers are provided on the websites of the province offices.

Upon the receipt of a permanent resident card for the family member of the EU citizen, the following have to be submitted:

  • a valid travel document and
  • a proof of incurring a fee of 30 zlotys.

A permanent resident card may be received by the applicant’s proxy. In such case, at the moment of receipt the proxy shall present, in addition to the abovementioned documents, the power of attorney granted by the applicant together with the proof of having paid the stamp duty on the power of attorney in the amount of 17 zlotys. Stamp duty on the power of attorney is payable on the bank account of the province office, competent as of the place of submitting the power of attorney (the bank account numbers are available on the websites of these offices). Granting the power of attorney is not subject to stamp duty when the proxy is a spouse, a parent, grandparent, a child, grandchild or sibling of the foreigner.

A permanent resident card for the family member of the EU citizen is valid for 10 years.

Thanks to a permanent resident card, the family member of the EU citizen can stay on Polish territory and he is allowed to cross the Polish border numerous times without a visa (with a valid travel document).
