An application to confirm the right of a permanent residence of the EU citizen

An application for a document confirming the right of a permanent residence of the EU citizen must be filed in person at the province office, competent as of the place of residence of the foreigner (EU citizen). The application must be filed on the official form in Polish language.

The issuance of the document confirming the right of a permanent residence of the EU citizen is subject to a fee of 30 zlotys. The fee is payable to the account of the province office where the application is submitted to. The numbers of bank accounts of province offices are available on their websites.

Upon the receipt of the document certifying the right of a permanent residence, the EU citizen shall provide:

  • a valid travel document or another valid document confirming the identity and citizenship (e.g. the identity document);
  • the receipt of having paid the fee of 30 zlotys.

 The document confirming the right of a permanent residence can also be received by the applicant’s proxy. In such case, at the moment of receipt the proxy shall present, in addition to the abovementioned documents, the power of attorney granted by the applicant together with the proof of having paid the stamp duty on the power of attorney in the amount of 17 zlotys. Stamp duty on the power of attorney is payable on the bank account of the province office, competent as of the place of submitting the power of attorney (the bank account numbers are available on the websites of these offices). Granting the power of attorney is not subject to stamp duty when the proxy is a spouse, a parent, grandparent, a child, grandchild or sibling of the foreigner.
