Issuing a new permanent resident card for the family member of the EU citizen

A permanent resident card for the family member of the EU citizen is to be replaced in the following cases:

  • the change of the data contained in it (e.g. the change of name);
  • damage or other circumstances that impede the identification of the holder;
  • expiry date.

If the permanent resident card for the family member of the EU citizen had been lost, a new card is being issued.
The replacement or the issuance of a new permanent resident card for the family member of the EU citizen shall take place immediately – upon the application of the family member who is not the EU citizen.

The application is submitted in person on the official form in the province office, competent as of the applicant’s place of residence. The application must be completed in Polish. The application for the replacement of the permanent resident card for the member of the family of the EU citizen shall be attached with the existing permanent residence card (in the situation of changed data or a damaged card).

A permanent resident card for the family member of the EU citizen is issued or replaced after having paid the prescribed fee. The fee amounts to 30 zlotys and it is payable to the bank account of the province office where the application is submitted to. The bank account numbers of the province offices are available on their websites.
